Ha Ha - actually in San Francisco hanging out in the 'Loin I've witnessed the same thing a few times, and people's reactions are pretty much the same. I think ignoring the weirdo crazies no matter how absurd the situation is much more a part of US city life than in Shanghai... The Olympic propaganda banner is a nice touch.

I think most of the folks walking by are well aware of what he's doing. One guy gives him a 'pathetic loser' look and several step into the street to avoid walking near him. In China they mostly only stare at laowai. Staring at other locals is considered highly impolite. That being said it is a flagrant case of public indecency. In a Western country there would be a crowd gathered around him egging him on, chastising him or critiquing his technique.
